PLEASE READ: This form is essential for a number of reasons:

1) It is an agreement that you are clear on the dates and times of the shoot, and the location. This will have been discussed with you; ask if unsure.

2) You have my contact number, and I will have yours. There will be no excuse for a lack of communication before the shoot if problems occur or you wish to discuss anything.
If you cancel a shoot, giving at least 48hours notice then I will not leave negative feedback. If cancel during the 48hours prior to the shoot then you must ring me and speak to me personally. If you fail to ring, and 'no show' then I will leave negative feedback.

3) Levels: I believe that any model and photographer should have agreed levels to work to for a shoot. Some shoots are simply fashion shoots, or portrait shoots; others, such as glamour, will need both the model and the photographer to be clear as to what is the maximum level that will be worked upto for the shoot.
